What are the most common reasons companies struggle or fail during ISO audits? (9 views)

Our company is preparing for an ISO 9001 certification audit later this year, and one thing I've been trying to understand is what actually causes companies to run into serious problems during the audit process.
Most of the articles and guides I find focus on how to achieve certification, but very few talk about the situations where businesses don't meet the requirements or end up facing major nonconformities. That makes me wonder whether there are common mistakes that organizations repeatedly make without realizing it.
We're a growing company, and while we've spent a lot of time documenting processes and improving consistency, I'm concerned that there may be gaps we aren't seeing because we're so close to the project. Sometimes things look fine internally until an outside auditor reviews them from a different perspective.
I'm especially interested in hearing from people who have experienced difficult audits firsthand, whether in their own company or as part of an audit team. Were the biggest issues related to documentation, lack of employee awareness, poor record keeping, management involvement, corrective actions, or something else entirely?
I'm not asking because I'm expecting our audit to go badly. I just think it's useful to understand where companies most often get into trouble so we can focus our efforts on the areas that genuinely matter. Learning from other organizations' experiences seems more valuable than relying solely on generic preparation guides.
If you've seen companies struggle during certification audits, what were the underlying reasons, and were those issues things that could have been identified much earlier in the process? I'd appreciate any real-world insights from people who have gone through it.
